Context: Chatterjee2024 is the first paper in the wiki to measure ANA in acute dengue using the gold-standard HEp-2 IIFA platform. Its most important contribution is not the absolute 54.8% rate (which is setting-specific and partly reflects HEp-2 sensitivity) but the IIFA→LIA gap: only ~34% of IIFA-positive dengue patients confirmed by LIA. This quantifies for the first time what the mechanistic literature suggested — dengue massively perturbs non-specific nuclear autoimmunity, but very little of this corresponds to the targeted specificities of established systemic autoimmune diseases. This is coherent with Shih2023’s population-level null finding.
The MCTD/myositis signal is the paper’s other key contribution. It diverges from Shih2023’s ADEM signal, raising the possibility that dengue may have multiple narrow autoimmune sequelae (neurological → ADEM; rheumatological → MCTD/myositis) that appear in different study populations/settings. Both signals are hypothesis-generating rather than confirmed: Shih2023 is large but may lack power for rare rheumatological outcomes; Chatterjee2024 has wide CIs and a hospital-enriched sample. A definitive study would need Shih2023-scale population coverage with LIA-based outcome ascertainment.

Its primary finding — that only ADEM is elevated after dengue, and only in the first month — substantially constrains the “dengue causes chronic autoimmunity” narrative that was supported primarily by case reports, the Garcia2009 Cuban cohort (autoimmune markers but not clinical diseases), and the Li et al. clinical cohort (shown here to have 51.4% misclassification). The Garcia2009 data (elevated ANA, IC, CRP at 2 years) remains unexplained by the Shih2023 population-level data, creating a productive new contradiction: biological autoimmune activation does not reliably produce clinical autoimmune disease.
